Ayurveda wellness services for pain-free life and optimal health

The following forms are made available online for new clients to fill out prior to the initial Ayurveda consultation. Please download the following two pdf forms, edit them with your responses as appropriate and email the completed forms to info@sri-ranga.com no later than 24 hours prior to the appointment in order for the appointment to be honored. Not submitting the forms in time will amount to appointment cancellation and shall incur cancellation fee. 

The forms are intended for new clients seeking Ayurvedic services only; existing clients need not fill out the forms unless your last appointment was at least a year ago.
